Immunization Information

In an effort to maintain a healthy campus, 果酱视频 has designated both required vaccines and highly recommended vaccines. These vaccines address diseases that are often seen on college campuses including COVID-19; measles, mumps, rubella; chickenpox; tuberculosis; whooping cough; meningitis; and influenza. Students are encouraged to satisfy 果酱视频 immunization requirements before arriving on campus. The 果酱视频 Health and Counseling Center (HCC) can also administer most of the required and highly recommended vaccines to students once students are enrolled and attending classes.

Required Vaccines for Students

Immunization for measles is required by the State of Oregon for all students. immunization for COVID-19 is required by 果酱视频 for all students prior to the start of the fall academic term.

Proof of Immunization Required

Official documentation (e.g. vaccination card, medical record) of all required doses of a . Current vaccinations are considered fully effective on the 15th day after the last dose. 

Official documentation (e.g. vaccination card, medical record) demonstrating one of the following: 

  • Two doses of MMR vaccine at least 28 days apart, if all doses administered after age 1;
  • Three doses of MMR vaccine at least 28 days apart, if first dose administered before age 1; 
  • Two doses of individual measles vaccine at least 28 days apart; 
  • IgG titer (blood test) showing immunity to measles
  • Date of birth prior to 1957

A hold will be placed on the registration process of students who are not compliant with measles, mumps, and rubella (MMR) requirements. 

Highly Recommended Vaccines for Students

The following vaccines are highly recommended for all students:

  • series completion, including boosters;
  • ;
  • series completion;
  • and series completion;
  • series completion (three doses);
  • (annual);
  • (PCV) or (PPSV) Students with certain medical conditions (e.g., severe asthma, diabetes mellitus, chronic liver or kidney disease) should discuss first with a clinician or healthcare provider.

All documentation must be translated into English before submission.

If you need to request records from a health care provider, download the Release of Information-External form from the (Forms > Release of Information-External) to do so. You may complete this form and submit it directly to your healthcare provider or school, which will give them permission to send the information directly to the HCC.

If you are unable to provide proof of immunity to measles, you will be required to have either a blood test to measure your immunity (called a titer), or receive the vaccine. This can be done at the HCC, or with an outside healthcare provider. Outside providers should assist you with completing a Certificate of Immunization status, located in the (Forms > Certificate of Immunization Status), which can then be provided to the HCC as proof of immunity. 

Requests for Exemptions/Waivers from Immunization Requirement

Students may request an exemption from immunization requirements for medical or non-medical reasons. Please be aware that, based on guidance from public health authorities, students who waive vaccine requirements may have other requirements or be excluded from campus during some disease outbreaks.

Waivers for Measles, Mumps, Rubella (MMR) Vaccine

To request a medical or non-medical waiver for the measles, mumps, rubella (MMR) vaccine:

  1. Download the Measles/MMR Exemption form from the (Forms > Immunization Exemption Forms > Measles/MMR Exemption) 
  2. For medical exemptions: 
    1. Have the form completed and signed by a licensed medical provider (MD, DO, ND, NP, PA). 
    2. Upload the signed form to the under the Upload tab.
  3. For non-medical exemptions: 
    1. Watch the Oregon Health Authority’s and print out the  certificate of completion.
  4. Upload BOTH the measles module certificate of completion and the signed Exemption request form to the HCC, via the (Forms > Upload).

Exemptions for COVID-19 Vaccine

To request a medical or non-medical exemption for the COVID-19 vaccine:

  1. Download the COVID Vaccine Exemption form from the  (Forms > Immunization Exemption Forms > COVID Vaccine Exemption)
  2. For medical exemptions:
    1. The student must complete section 1 and sign section 4 of the form.
    2. Have section 2 completed and signed by a licensed medical provider (MD, DO, ND, NP, or PA) indicating the medical reason for not receiving the COVID vaccine/s.
  3. For a non-medical exemptions:
    1. The student must complete section 1 and sign section 4 of the form.
    2. Have section 3 completed and signed by a licensed medical provider (MD, DO, ND, NP, or PA) that they have reviewed with the student the risks and benefits of the COVID vaccine and the risks associated with contracting COVID-19.
  4. Upload the completed and signed form with any supporting documentation to the HCC, via the under the Upload tab.

Other Health Record Requirements

All students must also upload a provider health assessment form, a health history, emergency contact form and complete a TB Screening questionnaire. Details about these forms and instructions for uploading them can be found at the Health Record Requirements page. These items must be completed prior to the first day of classes in the fall.
